IUI is one such entry-level line of treatment that even the best fertility clinic in India suggests to couples who have failed to conceive a child after regular and unprotected sexual intercourse for a year or thereabouts.

What is IUI Treatment?

Also known as intra uterine insemination, IUI is a type of artificial insemination technique where the doctor transfers washed and prepared sperm into a woman’s womb (uterus) and in proximity to the egg during ovulation. The procedure is usually offered together with ovulation inducing drugs, which increase the chances of pregnancy, and Trans Vaginal Ultrasound (TVS), which helps in predicting ovulation.

Who is IUI Intended For?

Intra uterine insemination is offered as a solution in the following cases,

  • Endometriosis – For infertility caused due to endometriosis of varying degrees, the first line of treatment usually involves ovulation induction for obtaining the best eggs and releasing sperm through IUI.
  • Donor Sperm – Women who choose to get pregnant using donor sperm mostly undergo IUI to get pregnant.
  • Male Partner Infections – If the male in the couple has HIV or Hepatitis, the sperm can be obtained and washed repeatedly with culture media to remove the serum, which makes it free of infections. This eliminates the risk of transmitting any infection from the male partner to the female partner.
  • Unexplained Infertility – In cases where the exact cause of infertility cannot be established, the first line of treatment starts with IUI and ovulation inducing fertility drugs.
  • Mild Male Infertility – Also known as subfertility, it’s when the semen analysis of the male partner shows the sperm to be of average concentration, have poor movement or structurally abnormal. An IUI treatment procedure can help in such cases to an extent as the procedure separates good quality sperm from the bad ones.
  • Cervical Factor Infertility – During ovulation, the cervix produces mucus which is supposed to facilitate easier movement of the sperm from the vagina to the fallopian tubes. But if this mucus is abnormally thick, it makes the sperm’s journey difficult. With IUI, the cervix is bypassed as the sperm are directly deposited into the uterus, which means a larger number of sperm can meet the egg.
  • Inadequate Cohabitation – There are cases where either one of the partners is away from home for extended periods of time. Using IUI treatment, the male partner’s sperm can be produced and preserved before being transferred to the female’s uterus during ovulation.
  • Semen Allergy – In rare cases, a woman might be allergic to the protein in her partner’s semen. So when her partner ejaculates into her vagina, it can result in burning, swelling and redness of areas that come into contact with the partner’s semen. For women who are severely sensitive to their partner’s semen, IUI is useful as it helps remove the semen’s protein content before placing it into the uterus.

How is the IUI Process Performed?

The semen sample produced by the male partner is washed in a special culture and filtered using specific methods. This helps to isolate the best quality sperm for the transfer. The IUI position is quite similar to that of a Trans Vaginal scan. A sterilized catheter which features a syringe is then used to draw the washed sperm sample and introduced into the woman’s cervix. At this point, the doctor injects the sperm into the woman’s uterus before taking out the catheter and withdrawing the speculum. The woman can resume her daily routine from this point onwards. No bed rest is required after the procedure.

What is the IUI Treatment Success Rate?

The success rates of intra uterine insemination are directly proportional to the number of cycles a woman is willing to undergo, which is similar to how the IVF success rate in India is influenced by the number of IVF cycles a woman is ready to undergo. Furthermore, due to its uncomplicated nature, the average cost of IUI is quite low. This makes it possible for couples to undergo multiple IUI cycles without incurring too much expense.

  • With one IUI cycle only, a woman has a 10 to 20 percent chance of achieving pregnancy.
  • With 3 to 6 IUI cycles, the chances of achieving pregnancy can go as high as 80 percent.
  • IUI’s pregnancy rate is similar to, or marginally lower than, that of fertile couples who achieve pregnancy without any assistance.

There are also certain factors that influence the success of IUI. These include,

  • The woman’s age. The IUI success rates by age vary significantly.
  • The sperm sample (among men with a low sperm count, IUI treatment success is limited).
  • The status of the woman’s fallopian tube (at least one tube should be open).
